Technical Assistance:
Discussed with center medication dosage that may be confusing as the dosage is written in milligrams and not in milliliters. Please follow up with the child's parent's to ensure correct dosage.

Comments:
An unannounced monitoring inspection was conducted today from 12:15 pm until 2:00 pm. There were 68 children observed directly supervised by 9 staff. The physical plant, 7 staff records, 7 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed eating breakfast for lunch, reading quietly on sleeping mats preparing for nap time, learning Halloween songs in music class, bullring animal habitats and playing outdoors. Teachers were observed managing transitions from the classroom to the playground and participating in conversations with children to include positive redirection. There was an abundant supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22VAC40-185-140-A
Description: Based on record review, one of seven children's records reviewed did not contain documentation of a physical examination by or under the direction of a physician within 30 days of the child's first attendance.
Evidence:
The Commonwealth of Virginia School Entrance Health Form in the record for Child 5 did not have Page 4 completed which is to be completed by a physician. Child's %'s date of enrollment is documented as 8/21/2018.

Plan of Correction: We will ask the parents to provide the information.
