A Subsidy Health and Safety inspection was conducted from 2:40 PM to 4:45 PM on October 17, 2019 at the school site and from 1:55 PM to 3:25 PM on a staff record review on October 18, 2019 to review supplemental health and safety requirements. Today, there were twenty-four school age children with four staff after school on this early release day. It was reported that twelve children were supervised by two staff before school. I viewed operational responsibilities, record keeping, caregiver training, emergency planning and drills, hazardous substance practices, behavioral guidance, five records for children, ten records for staff, nutrition and food service. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. No violations of the supplemental subsidy health and safety requirements were found. Let me know if you need any assistance. 540/430-9259
A compliance history is in no way a rating for a facility.
The online compliance history includes only information after July 1, 2003. In addition, the online compliance history includes information regarding adverse actions that may be the subject of a pending appeal. An adverse action is not final until a provider has exhausted or waived all due process rights. For compliance history prior to July 1, 2003, or information regarding the status of pending adverse actions, please contact the Licensing Inspector listed in the facility's information.
