Comments:
An unannounced renewal inspection was conducted today 10/18/2019. The inspection began at 11:00 am and concluded at 1:30 pm. There were a total of 28 children present with seven staff members. During the inspection five children's, and five staff's records were viewed, two buildings were viewed, emergency supplies were viewed, infant care was observed, diapering was observed, food service was viewed, emergency supplies were viewed and polices and procedures were reviewed. The center does not administer any medication per policy and does not provide any transportation.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on review of five children's records, the center failed to ensure that all physical exams were obtained within the 30 days of enrollment.

Evidence:
1. Child # 1 enrolled into the program on 3/4/2019 and the date of the physical exam was 9/4/2019.

Plan of Correction: In the future the director will have all physical exams within the 30 days and will monitor all dates.

Standard #: 22VAC40-185-240-B
Description: Based on review of staff's written policies and procedures, the center failed to ensure that all policies required by the standard were completed in writing and given to staff by the end of their first day.

Evidence:
1. The written staff's policies and procedures were missing the following: Procedures for supervising a child who may arrive after scheduled classes or activities including field trips have begun and procedures for identifying where attending children are at all times, including procedures to ensure that all children are accounted for before leaving a field trip site and upon return to the center;

Plan of Correction: The director will add the two missing policies and will ensure that all staff received the new version in writing.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center failed to ensure that all hazardous substances were kept in a locked place using a safe locking method.

Evidence:
In the toddler room, the locking mechanism on the cabinet below the diaper changing table did not prevent entry into the cabinet. There were several containers of cleaning chemicals that were stored in the cabinet.

Plan of Correction: During the inspection the director replaced the locking mechanism and will have staff to monitor.

Standard #: 22VAC40-185-570-E
Description: Based on observation, the center failed to ensure that all prepared infant formula was refrigerated, dated and labeled with the child's name.

Evidence:
1. Child # 6 bottle of prepared infant formula was dated for yesterday 10/17/2019.

Plan of Correction: The director will discuss with staff about rechecking all bottles as they are made or used.
