Comments:
An unannounced monitoring inspection was conducted between the hours of 8:00 am and 10:30 am. Upon arrival there were 65 children in care with 15 staff present A sample of 3 staff and 6 children's records were reviewed in addition to 3 medications. Children were observed in learning centers and listening to music.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based on a record review the center did not ensure all required information was kept for each staff person.
Evidence:
Staff #3 (hired 02/18/19) did not have the correct job title documented in the file. Staff #3's job title was documented as a program leader. The Director confirmed Staff #3 was promoted to Assistant Director on October 7, 2019.

Plan of Correction: The following corrective action was provided by the Director during the exit interview:
This was an oversight. During the inspection the Director documented the staff member's job title and promotion date in the file.
