Standard #: 22VAC40-185-80-A Description: Based on review of documentation and staff interview. the Facility did not maintain an accurate written record of children in attendance.
Evidence: In Classrooms 2.4.and 6 the written attendance did not match the number of children present during the inspection.

Evidence: Child 4 has an Auvi-Q medication for food allergies but there is no documentation of staff having the training to give the medication.
Plan of Correction: We will have someone complete the training today and then will have more staff take the training as well.
Standard #: 22VAC40-185-270-A Description: Based on observation the Facility did not maintain all outdoor areas in a safe condition.
Evidence: On the Toddler and School age playgrounds there were multiple areas where the black weed stop under the mulch was sticking up through the mulch posing a tripping hazard.
Plan of Correction: We will add more mulch to the playground.
Standard #: 22VAC40-665-670-G Description: Based on review of written documentation and staff interview, tummy time is not being documented daily.
Evidence: There was no written documentation of tummy time during the inspection and the staff reported that they had not documented it yesterday or today.
Plan of Correction: We have a form to document tummy time so we will monitor for the next week to make sure they are doing the paperwork.
