An unannounced monitoring inspection was conducted today from 10:10am until 11:15am. There were 15 children in care (all 3-years-old) being supervised by two staff. Children were observed finishing circle time, playing outside, washing hands, and having snack. Snack is provided by the parents. Five children's files, two staff files, and five board members files were reviewed. The first aid kit, battery operated radio, and flashlight were all inspected and in compliance. There was no medication on site for children in the morning program. The last evacuation drill was conducted 9/10/19 and the last shelter-in-place was conducted 5/17/19.

Standard #: 22VAC40-185-130-A Description: Based on a review of five children's files, the Facility did not have documentation of one child's immunizations by the first day of attendance.
Evidence: Child 1 (first date of attendance 9/3/19) did not have documentation of immunizations in the record.Plan of Correction: The staff stated that the family is going to the doctor tomorrow and will bring the immunizations when the child returns Thursday.
