Comments:
This facility is currently on a probationary status.
An unannounced monitoring inspection was conducted today. All rooms are in ratio during the inspection. 2 staff and 5 children's records were reviewed. All were complete. Children were observed washing hands, eating snack, playing with toys and during diapering. Medications, emergency supplies, first aid supplies and menus are all in compliance. A Subsidy Health and Safety Inspection (SHSI) was also conducted today.

Violations:
Standard #: 22VAC40-185-260-A
Description: Based on a review of available documentation, there has not been an annual fire inspection conducted at the center.
Evidence:
The inspection report available for review is dated 6/8/18.(Expired)

Plan of Correction: I have contacted the fire marshal's office. We are scheduled for an inspection on 10/29/2019

Standard #: 22VAC40-185-270-A
Description: Based on direct observation, the infant suite has not been maintained in a clean, safe and operable manner.
Evidence:
There is peeling pain and holes on the wall on the older infants side, within reach of the children.

Plan of Correction: We put contact paper over the area, and rearranged the room to cover the wall area. I will contact Fairfax County facilities to have it repaired.

Standard #: 22VAC40-185-280-H
Description: Based on direct observation, purses were not stored in an area not accessible to children.
Evidence:
In the Panda's room, the closet where staff store their purses was not locked.

Plan of Correction: We will install a lock on the door.

Standard #: 22VAC40-185-510-J
Description: Based on direct observation, one medication was not stored locked.
Evidence:
One Epi-pen in Panda's room was not stored locked. The medication was in the closet, in a backpack, neither was locked.

Plan of Correction: Medications will be stored locked. We will order safety sacks for medications.

Standard #: 22VAC40-185-550-E
Description: Based on a review of documentation, the record of dates for practiced emergency evacuation drills was not maintained.
Evidence:
There is no documentation of an emergency evacuation drills having been conducted in the month of July 2019

Plan of Correction: I will put a computer reminder on my computer to remind me to document the drills.

Standard #: 22VAC40-185-550-M
Description: Based on a review of injury reports, all required components are not being documented.
Evidence:
8 of 8 reports reviewed did not contain documentation of the date or the time of parent notification.

Plan of Correction: We will retrain on accident reports. A highlighted sample sheet for all (specifically high-lighting time and date) for all to follow.
