Technical Assistance:
Discussed with Director: medication authorization for Over-The-Counter (OTC) medications, Soccer Tots coaching staff file requirements.

Comments:
An unannounced monitoring inspection was conducted today. all rooms are in ration during the inspection. Children were observed washing hands, transitioning to the outside, during circle time. being fed and while resting. 2 staff an 4 children's records were reviewed. All were complete. Area of non-compliance is cited on the violation page. First aid supplies, posted documents. emergency supplies and medications are in compliance. LI was present today from 10:25 AM - 1:10 PM.

Description: Based on a review of injury reports, all required components are not documented on the reports.
Evidence:
3 of 6 reports reviewed did not include the time of parent notification.

Plan of Correction: All staff will be reminded to complete all areas of the report form.
