Comments:
A Subsidy Health and Safety inspection was conducted on October 18 2019 to review supplemental health and safety requirements. 26 children were observed in the direct care of 5 staff members. Children were observed reading, playing outside, and participating in various games. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description: Based on record review, 1 of 5 children's records reviewed did not have instructions to from a physician of to be taken in the event of a suspected or confirmed allergic reaction .

Evidence: Child # 4 record does not have an allergy action plan for the child's diagnosed allergy.

Plan of Correction: Staff has requested the parent receive the information from the doctor. We will follow up with mom today.
