Comments:

A Subsidy Health and Safety inspection was conducted on October 16, 2019 from approximately 10:00 AM to 1:20 PM to review supplemental health and safety requirements. The following staff-to-child ratios were observed in the classrooms: Infants 3:10, Toddlers 2:8, 2 year olds 2:16, 3-5 year olds 2:17. The staff were observed to be engaged with the children in age appropriate activities and to be meeting the needs of the children in care. Most, but not all areas, of the facility were inspected. The facility had the required postings in the facility. The first aid kit and non-medical emergency supplies were inspected and found complete. Emergency drill records were found complete. Medications and authorizations were reviewed. 6 children's records, 5 staff records, and 2 agent records were reviewed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Please complete the "plan of correction" and "date to be corrected" for each violation cited on the violation notice and return it to the licensing office within 5 business days. Your plan of correction should contain 1) steps to correct the non-compliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Standard #:	22VAC40-665-540-A
Description:	Based on a review of five staff records, the center did not ensure that three records contained documentation of a negative tuberculosis screening at the time of employment and before coming into contact with children. Evidence: 1. The licensing inspector observed the record for Staff #1 (hire date 9/30/19) contained a tuberculosis screening dated 10/11/19. 2. The record for Staff #2 (hire date 9/13/19) did not contain a tuberculosis screening. 3. The record for Staff #3 (hire date 8/22/19) contained a tuberculosis screening dated 9/13/19.
Plan of Correction:	Management will ensure that all new hire staff obtain a TB test before their start date.
