Technical Assistance:
Discussed the playground flooring.

Comments:
An unannounced monitoring inspection was conducted from 8:42am-12:45pm. At the time of entrance, 92 children were in care with 22 staff members present. Children were observed in the fitness room, painting bats, playing with puzzles, listening to flannel stories, listening to the teacher read a book, preparing for lunch, singing, playing with toys in the sand table, creating geo shapes, lacing string on a teeth art project, working on computers, ink stamping their name, playing in centers and outdoor play time. The site was clean, organized and contained an abundant supply of developmentally appropriate materials. Interactions between the children and staff were positive. A selection of staff and children records, the physical space, emergency supplies, evacuation drills, attendance records, injury reports, medications and the Emergency Preparedness Plan were reviewed. . Standard #: 22VAC40-185-160-A
Description: Based on review of 10 staff records, the center did not have documentation of a negative tuberculosis screening within 21 days after employing or within 12 months prior to hire.
Evidence:
1. Staff #9 (date of hire 8/19/2019) and Staff #10 (date of hire 6/24/2019) did not have documentation of a negative tuberculosis screening in their file within 21 days of employment.

Plan of Correction: We will ask them to get that done today.

Standard #: 22VAC40-185-160-C
Description: Based on record review, the center did not obtain updated tuberculosis tests or screenings for all staff members every 2 years.
Evidence:
1. Staff #1 (Date of hire 3/01/2004) had a tuberculosis test dated 3/22/2017 and needed an updated test or screening for the staff file.

Plan of Correction: We will ask her to get it done.

Standard #: 22VAC40-185-270-A
Description: Based on observation, equipment inside of the center were not maintained in a safe and operable condition.
Evidence:
1. In classroom #4, the blue chair had a small hole in the fabric on the right corner..
2. In classroom #5, the blue sofa had a small hole in the fabric.
3. In classroom #6, the red chair had a small hole in the fabric on the right side.
4. In classroom #7, the blue sofa had a small hole in the fabric on the right corner.

Plan of Correction: We will contact the manufacturer to see if there is a warranty and if not, replace them with something.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances such as cleaning supplies were not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. In classroom 8, a spray bottle of bleach water was inside an unlocked cabinet.

Plan of Correction: The bleach water was moved immediately to a locked closet.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on record review, the center did not obtain a copy of the results of a search of the central registry by any other state in which the individual has resided in the preceding five years.
Evidence:
1. Staff #9 (Date of Hire 8/19/2019) lived in TX in the past 5 years and the center did not send for the central registry results for that state.

Plan of Correction: The central registry for staff #9 was sent during the inspection.
