Comments:
An unannounced renewal study was conducted today from 10:00-12:00. All rooms were in ratio with direct care staff. Reviewed physical space, a sample of children's records, new hires, emergency preparation, trainings and care of children. Observed children in circle time, transition to the outside playground, soccer enrichment, reading, art and lunch. Lunch served today was tacos with lettuce, tomato and cheese. Standard #: 22VAC40-185-130-B
Description: Based on record review, 1 of 6 records did not contain documentation of an immunization update every six months for children under the age of two.
Evidence:
Child #4's last immunization update was 3/1/2019.

Plan of Correction: Child #4 received updated immunizations.

Standard #: 22VAC40-185-60-A
Description: Based on record review, 4 of 6 records did not contain all required information.
Evidence:
1. Child #1 and Child #2's proof of identity was missing the signature and date of the individual viewing the information.
2. Child #5 (last update 104/2018) and Child #6 (last update 10/19/2017) records did not contain documentation of an annual update..

Plan of Correction: Child #1 and #2 signatures have been obtained. Child #5 and #6 both families made appointments for physicals.

Standard #: 22VAC40-185-270-A
Description: Based on observation, several small areas in the center were not maintained in a clean, safe or operable condition.
Evidence:
Paint was popping in 3 areas: the bathroom in the Barcelona class, the hallway leading downsairs to the lower level and in the preschool room by the sink.

Plan of Correction: Touch up painting is scheduled.

Standard #: 22VAC40-185-500-B
Description: Based on observation and staff statements, diapering supplies are not in use for the Tokyo classroom.
Evidence:
A diapering surface that can be clean and sanitized as well as a foot operated trashcan to dispose of wet diapers is not in the Tokyo bathroom where wet diapers are changed.

Plan of Correction: All changing supplies are centrally located to include stip on trashcan and changing mat.
