Technical Assistance:
Due to the background check related violations cited in today?s inspection, the provider is required to follow-up with the appropriate department regarding the status of any missing and/or incomplete background checks. Documentation of the completed follow-up must be sent to the Licensing Inspector within 10 business days, and a copy of all missing/incomplete background check results must be sent to the Licensing Inspector upon arrival.

Discussed outlet covers and staff proof of education.

Comments:
An unannounced monitoring inspection was conducted today from 12:30pm-2:30pm. There were 34 children (2-5yrs.) supervised by 4 direct care staff. The physical plant, 2 staff records, 4 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed napping. There was a sufficient amount of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22VAC40-185-160-C
Description: Based on a review of staff records, 1 of 2 staff records did not contain documentation of a current report of TB screening at least every two years from the date of the first initial screening or testing.

Evidence: Caregiver A's (date of hire: 9/9/2019) most recent documented TB test was dated 10/13/2017 (expired: 10/13/2019).

Plan of Correction: The employee will get their TB done as soon as possible.

Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records, 1 of 4 children's records did not contain documentation of all required information.

Evidence: Child #2's (3 years old) record did not include documentation of proof of identification and previous schools attended.

Plan of Correction: We will give the parent the missing page and have them fill in missing information.

Standard #: 22VAC40-185-280-B
Description: Based on observation, 1 of 2 classrooms did not ensure that hazardous substances were kept in a locked place using a safe locking method that prevents access by children.

Evidence: In the St. Mary's classroom (children ages 4-5 years), a box of baking soda was found in an unlocked cabinet in the boy's bathroom.

Plan of Correction: Corrected during inspection.

Description: Based on record review, 1 of 2 injury reports did not include documentation of all required information.

Evidence: One injury report did not include documentation of the time of parent notification.

Plan of Correction: We will fill in missing information.

Description: Based on a review of staff records, 1 of 2 staff records did not contain documentation of a central registry findings within 30 days of employment.

Evidence: Caregiver A's (date of hire: 9/9/2019) record did not contain documentation of a central registry request or findings.

Plan of Correction: It has already been sent off and we will follow-up.
