Comments:
An unannounced monitoring inspections was conducted today between 9:30 am and 12:30 pm. There were 10 children present with 2 staff members assigned to their direct supervision. In addition there was 1 actively involved co-op parent volunteering on site today. The children were observed hand washing, during circle time and during free play, the were also observed eating snack. Snack consisted of string cheese, ritz crackers, apples and water. The physcial plant, children's records, staff records, emergency plans, Incident/Accident reports, evacuation drill logs, attendance logs, emergency supplies and medications were inspected. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records, the center failed to ensure that children's records contained required information.

Evidence:
1) Child #1,#2,#3,#4 and #5's records do not contain documentation of child's first day of attendance.
2) Child #2 and #3's records do not contain documentation of either parents work phone number and place of employment.
3) Child #1's record does not contain documentation of a work phone number for the mother.

Plan of Correction: I will get the required information from parents and place in children's records.

Standard #: 22VAC40-185-290-3
Description: Based on observation, the center failed to ensure that all electrical outlets have protective covers that are of a size that cannot be swallowed.

Evidence:
The outlet in the women's bathroom does not have protective covers.

Plan of Correction: Will put safety covers in bathroom outlet.

Standard #: 22VAC40-185-510-G
Description: Based on a review of medications, the center failed to ensure that written authorization from the parent was obtained for medication.

Evidence:
Child#1's record did not include authorization to administer emergency medication.

Plan of Correction: I will get authorization form from parents.
