Technical Assistance:
LI had a discussion with with program owner during last inspection on 6/3/2019 regarding acquiring adequate staffing in the event that an employee calls out sick/is on vacation. Additional staff has not been hired and a staff person was out sick during today's inspection.

Comments:
Today an unannounced Monitoring Inspection took place between the hours of 10:00am and 12:35pm. At the start of the inspection there were a total of 11 children with 2 staff; ratios were not incompliance. A total of 5 children?s files and 5 staff files were reviewed. The children were observed eating a morning snack (goldfish and applesauce), coloring a ghost, dancing, singing in Spanish and other organized activities. Standard #: 22VAC40-185-130-B
Description: Based on record review, the center did not obtain documentation of additional immunizations once every six months for children under the age of two years.
Evidence: Child #1 (23 months) most recent immunizations are dated 2/11/2019. Child #5 (9 months old) most recent immunizations are dated 2/4/2019.

Plan of Correction: I'll let the parents know that I need a copy.

Standard #: 22VAC40-185-240-C
Description: Based on record review, the program director has not attended 16 hours of staff development activities annually.
Evidence: Staff #4 (date of hire: 8/2018) files did not contain documentation of any staff development training.

Plan of Correction: I did a training through my church but they didn't provide a certificate. I will make sure to complete the training the training for this year.

Standard #: 22VAC40-185-280-D
Description: Based on observation, cleaning and sanitizing materials were located next to food.
Evidence: In the cabinet above the food prep area there is an all-purpose cleaner located next to a basket full of food pouches.

Plan of Correction: I'll move the cleaner into another cabinet.

Standard #: 22VAC40-185-350-C
Description: Based on observation and record review, the center did not follow the staff-to-child ratio applicable to the youngest child in the group in a regularly mixed age group.
Evidence: At approximately 10:00am, there were 8 children present with one staff in the toddler/preschool classroom. The children's records showed that the youngest child in the room was 21 months old. The ratio for a 21 month old is 1 staff to every 5 children.

Plan of Correction: I thought that if I was in the other classroom with the door open that the youngest child could count as being in my ratio. If in this position again I would bring one more child in the infant room with me.

Description: Based on observation, diapers were not disposed in a leak-proof or plastic-lined storage system.
Evidence: The trash can in the infant room used for diaper changes did not contain a plastic liner. There were diapers observed in the trash during today's inspection.

Plan of Correction: This was corrected during the inspection.

Standard #: 22VAC40-185-560-K
Description: Based on observation, tables and high chair trays were not sanitized before and after each use.
Evidence:
1. At the start of today's inspection the children were observed eating a morning snack. When the children were finished eating they started an art craft. The tables were not sanitized after eating.
2. The children were observed eating lunch. at the classroom tables and three children were in high chairs. The tables and high chairs were not sanitized before the children were served their lunch.

Plan of Correction: I'll make a sign to remind the teachers.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on record review, the child day center did not obtain an out-of-state central registry background check for an employee that currently resides outside of the state of Virginia.
Evidence: Staff #5 currently lives in the state of Maryland. There is no documentation of a Maryland state central registry background check in the employees file.

Plan of Correction: I'll make sure to do that.
