Comments:
On 10/18/2019 a self-reported incident regarding the interactions between a child and staff member was called into the Licensing office. An unannounced focused investigation was conducted today between the hours of 10:15am and 2pm. Based on the preponderance of evidence gathered, violations were cited. In addition, a Monitoring Inspection also took place today. At the start of the inspection there were a total of 8 children with 3 staff; all ratios were incompliance. A total of 6 children?s files and 5 staff files were reviewed. The children were observed doing puzzles, practicing cutting, playing outside and eating lunch (meatball subs with mozzarella cheese, green beans and apples) and other organized activities. Standard #: 22VAC40-185-160-C
Description: Based on record review and interview, staff have not resubmitted a negative tuberculosis (TB) screening every two years.
Evidence: The most recent TB screening on file for Staff #4 is dated 10/16/2017. Staff #4 has confirmed that they have not yet received a subsequent screening.

Plan of Correction: It is scheduled for next week.

Standard #: 22VAC40-185-60-A
Description: Based on record review, children's records did not contain all required information.
Evidence: Child #1 and Child #3's files did not contain documentation of an annual review of contact information.

Plan of Correction: I'll have them updated tonight.

Standard #: 22VAC40-185-240-D-5
Description: Based on record review and interview, there is not one staff member who has obtained the daily health observation training on duty at all times.
Evidence: Center management has confirmed that only one staff person has current daily health observation training. That staff person is not at the center at all times.

Plan of Correction: I'll look into getting two more staff with the training to make sure we always have coverage.

Standard #: 22VAC40-185-270-A
Description: Based on observation, equipment in the center was not maintained in a clean, safe and operable condition.
Evidence: A deflated bean bag chair was observed in the radiant rainbow classroom.The chair has several large holes and tears along the seams of the chair.

Plan of Correction: We'll get rid of it today.

Standard #: 22VAC40-185-400-B
Description: Based on interview, documentation review and photographic evidence, behavioral guidance was not constructive in nature.
Evidence: On 10/17/2017, Staff #5 was stopping Child #6 (3 years old) from hitting Child #1. Child #6 then began to swat at Staff #5. Staff #5 grabbed Child #6 by the hands in an attempt to stop Child #6. Staff #5 scratched Child #6 on both sides of the child's forehead while grabbing their hands. Child #6 had two nail scratches on the left side of their forehead and one on the right side of their forehead. All three scratches broke Child #6's skin.

Plan of Correction: I'll retrain the staff to reach out to a manager if an incident like this begins to occur.
