All record keeping information that is required by the Minimum Standards must be clearly documented in each child's record. If information is verbally provided by the parent, the addition of that information to the child's record must be initialed by the parent.
An unannounced monitoring inspection was conducted today. There were 70 children in care, with a total of 13 staff members assigned to their direct supervision. Children's records, new staff records, staff training, background checks, the physical plant, classroom interactions, and children's medications were inspected today.
This inspection took place in the middle of the day. All children were observed during morning class dismissal or preparing for their afternoon nap.

Standard #: 22VAC40-185-60-A Description: Based on a review of children's records, it was determined that the records for three enrolled children do not contain all required information.
Child records A, B, and C do not include each employed parent's work phone number.
Plan of Correction: Each parent will be asked for the information.
Standard #: 22VAC40-185-80-A Description: Based on a review of administrative records, it was determined that the center did not keep an accurate written of all children in attendance.
In Room 2, there were 10 children present and in care but the written attendance for today indicated a total of only 9 children present.
Plan of Correction: Staff will be reminded to keep an accurate record of attendance at all times.
