Technical Assistance:
If you plan to double ratios at nap time for children 16 months and older you must make sure that staff counted in the overall rest period ratio are within the building and available to ensure safe evacuation in an emergency and an additional staff person is at the center to help, if necessary. If these conditions cannot be met then you cannot double ratios at nap time ( please refer to standards 350.D 1-3)

Please review all standards regarding ratios and supervision to ensure that your remain in compliance at all times.

During the center's hours of operation, one adult on the premises shall be in charge of the administration of the center.( standard 340B).

Comments:
A complaint was received involving sight and sound supervision, feeding two infants out of the same bowl, failing to wash hands after diapering and failing to wash hands before feeding the infants. A review of video footage, observation and interviews were conducted on 10/4/19 and part of the complaint involving sight and sound supervision was determined to be valid.
Please submit your plan of correction by 10/21/19 and make sure your address in your plan how you plan to ensure on going compliance.

Description: Based on observation, it was determined that the center failed to maintain an accurate attendance log.
Evidence:
There were 18 children napping, (2 to 4 years of age ) in the preschool classroom however there were only 17 children written on the attendance log.

Plan of Correction: Teachers have been retrained on the importance of keeping accurate attendance. We will also, as manager, take the time to check attendance several ti mes throughout the day.

Description: Based on observation, it was determined that a staff left children under 10 years of age out of sight and sound supervision.
Evidence:
On 10/4/19 at approximately 12: 30 pm, staff A left eighteen, 2/3/4 year old napping children out of sight and sound supervision when the staff opened the front door for the Licensing Inspector.

Plan of Correction: Retrained staff on 10/7/19 on site and sound supervision and will be hiring a staff shortly so we will always have an available person to handle emergencies.

Description: Based on observation, it was determined that the center did not adhere to the doubling of ratios during a designated rest period in the 2/3/4 year old classroom.
Evidence:
The Licensing Inspector observed 18 children with 1 staff during nap time from approximately 12:30 pm until 1 pm. The children were 2, 3 and 4 years of age which required a ratio of 1:16 at nap time.

Plan of Correction: At the time we didn't understand nap time ratios but now we understand and will stay within ratios moving forward.

Description: Based on observation, it was determined that the most recent inspection, dated 9/19 was not posted at the center.

Plan of Correction: We will make sure to post the most recent visit immediately after each visit.
