TA: Guidance was given on how to submit a serious injury report on the DSS web site.

Comments:
A Subsidy Health and Safety inspection was conducted on 6/12/19 and 7/3/19 to review supplemental health and safety requirements in response to a self-reported incident regarding a child's injury that occurred on 5/6/19. Based on record review, staff interviews, classroom interview, review of written policy, review of injury report, and observation of the playground, violations are noted on the Violation Notice. A licensing inspection was also conducted on these dates; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the Violation Notice. Standard #: 22VAC40-665-550-2
Description: Based on record review and staff interview the vendor did not inform the department's representative within two business days of any injury to a child while under the vendor's supervision when a referral is made for treatment from a medical professional.
Evidence:
1. Child A sustained an injury to her chin, requiring treatment from a medical professional on 5/6/19, however, the Provider did not report the incident to the Licensing representative until 06/12/19.

Plan of Correction: We did not report the injury because we did know that we were required to report incidents like this. Moving forward we will report any injury that requires medical attention within 48 hours to the Department of Social Services.
