Comments:
An unannounced Complaint Inspection was conducted today in response to a complaint that was received by the Virginia Department of Social Services, Licensing Division. Interviews were conducted with staff relating to allegations of lack of supervision and lack of care and protection. Based upon a preponderance of evidence, the information gathered during the investigation supports the allegations, and the complaint is determined to be valid. Please complete the columns for 'description of action to be taken' and 'date to be corrected' for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office by November 8th, 2019. Complaint related: Yes
Description: Based on staff interviews and video recordings, the facility did not ensure that while supervising children, they shall always ensure their care, protection and guidance. Evidence: Identified that Child 1 fell while stepping off of the play equipment and hit their head. Staff 1 was nearby but did not see the incident, and was unaware of the incident until children came to them and told them that Child 1 was hurt. Since staff did not know that the child had hit their head, Staff 1 did not respond accordingly to a child with a head injury. Child 1 was "out of it" after the incident and vomited twice. Only when the parent picked up the child and noticed the bump on the child's head did Staff realize child was injured.

Description: Based on staff interviews and video footage of the alleged event, the facility did not ensure that children under 10 years of age are always within actual sight and sound supervision of staff. Evidence: Identified that Staff 1 was on the playground with 9 children and did not see Child 1 fall and hit their head on the play equipment. According to video, Child 1 sat down after the incident, garnering attention from other children. Staff 1 did not know about the child's injury until children had alerted them to the event.
