Technical Assistance:
Technical assistance was offered regarding group sizes and children's allergies.

Comments:
A Subsidy Health and Safety inspection was conducted on 10/18/19 to review supplemental health and safety requirements. There were 63 children with 12 staff members. The children were observed playing with toys, structured centers, coloring, playing outside and eating lunch. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Description: Based on records review, the center did not obtain Tuberculosis (TB) screenings for employees completed within the last 30 calendar days of the date of employment.
Evidence:
1. Staff # 1 was hired on 10/15/19. Staff # 1's TB screening was dated 11/29/18.
2. Staff # 2 was hired on 6/15/19. Staff # 2's TB screening was dated 10/15/18.
3. Staff # 3 was hired on 10/14/19. Staff # 3's TB screening was dated 5/23/19.
4. Staff # 4 was hired on 8/30/19. Staff # 4's TB screening was dated 12/28/18.

Plan of Correction: I will remember to obtain TB screening within 30 days of employment.
