Technical Assistance:
Technical assistance was offered regarding staff records and background checks.

Comments:
An unannounced monitoring inspection was conducted from 10:10 am-1:00 pm. 56 children were in care with 15 staff members present. Children were observed engaged in group and circle time activities, outdoor play time and eating lunch. The lunch menu included a turkey sausage burger, sweet potatoes, peaches and milk. 4 staff and 5 children's records, and other documents were reviewed. Violations:
Standard #: 22VAC40-185-140-A
Description: Based on record review, the center did not obtain documentation of a physical exam for children in care within 30 days of enrollment.
Evidence: Child # 1's first day of enrollment was 8/26/19. Documentation of a physical exam was not obtained for Child # 1 within 30 days of enrollment.

Plan of Correction: I will ask the parents to bring in a copy of the physical.

Standard #: 22VAC40-185-160-A
Description: Based on staff interview, the center did not obtain tuberculosis (TB) screening for an employee within 21 days of employment.
Evidence:
1. Staff # 1 is an independent contractor at started soccer sessions for children enrolled in the program. The soccer season started on 9/23/19.
2. Staff # 2 stated that the center did not have a TB screening for Staff # 1.

Plan of Correction: We will obtain a TB screening for Staff # 1.

Standard #: 22VAC40-185-70-A
Description: Based on observation and staff interview, the center did not maintain complete records for employees.
Evidence:
1. At 10:35 am 6 children, ages 2 years to 4 years old, were observed in a soccer class with Staff # 1 on the playground.
2. Staff # 1 was alone with the children, and was responsible for their supervision and ratios. There was no additional center staff on the playground with Staff # 1 and the children.
3. Staff # 2 stated that normally a center staff accompanies the group of children during the soccer sessions.
4. The center did not complete orientation with Staff # 1.
5. Documentation of 2 reference checks were also not obtained for Staff # 1 before the first session that started on 9/23/19.

Plan of Correction: Staff # 1 will be accompanied with a center staff moving forward.

Standard #: 22VAC40-191-60-B
Description: Based on observation and staff interview, the center did not obtain a sworn disclosure statement (SDS) from an employee before the first day of employment.
Evidence:
1. At 10:35 am 6 children, ages 2 years to 4 years old, were observed in a soccer class with Staff # 1, an independent sports contractor, on the playground.
2. There was no other center staff with the group of children and Staff # 1.
3. Staff # 2 stated that the first soccer session started on 9/23/19, and each session runs from 10:00 am to 10:30 am. Staff # 2 also stated that normally a center staff accompanies the group of children during the soccer sessions.
4. The center did not obtain an SDS from Staff # 1 before leaving her alone and in supervision of the children.

Plan of Correction: I will make sure that we send out a center staff for soccer sessions.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on observation and staff interview, the center did not obtain fingerprint result for an employee.
Evidence:
1. At 10:35 am 6 children, ages 2 years to 4 years old, were observed in a soccer class with Staff # 1, an independent sports contractor, on the playground.
2. There was no other center staff with the group and Staff # 1.
3. Staff # 2 stated that the soccer session was held from 10:00 am to 10:30 am. The center did not obtain fingerprint results for Staff # 1 before leaving her in direct supervision and ratio of the children.

Plan of Correction: We will make sure that there is always a center staff present with the soccer instructor.
