An unannounced monitoring inspection was conducted from 9:00 am-1:15pm. At the time of entrance, 129 children were in care with 28 staff members present. Children were observed during handwriting activities, listening to the teacher read a book, coloring, pasting pictures in sequence, painting, bathroom breaks, washing hands, fitness class, playing in centers, eating lunch and outdoor play time. Infants were observed being held, fed, playing with toys and sleeping. The site was clean, organized and contained an abundant supply of developmentally appropriate materials. Interactions between the children and staff were positive. A selection of staff and children records, the physical space, emergency supplies, evacuation drills, attendance records and medications were reviewed. . Standard #: 22VAC40-185-160-A Description: Based on review of 10 staff records, the center did not have documentation of a negative tuberculosis screening within 21 days after employing or within 12 months prior to hire.
Evidence:
1. Staff # 4 (date of hire 6/17/2019) did not have documentation of a negative tuberculosis screening in their file within 21 days of employment.Plan of Correction: Staff #4 has been instructed again to obtain her TB test immediately.
Standard #: 63.2(17)-1720.1-B-3 Description: Based on record review, the center did not obtain a copy of the results of a search of the central registry by any other state in which the individual has resided in the preceding five years.
Evidence:
1. Staff # 4 (Date of hire 6/17/2019) lived in AK in the past 5 years and the center did not send for the central registry results in that State.Plan of Correction: We will order the background check today.
