Comments:
An unannounced monitoring inspection was done today 10/22/2019 beginning at 9 AM and ending at 1 PM. There were 53 children present. Five children's and six staff's records were reviewed during the inspection. free play,classroom playtime, Lunch and nap were observed, and a physical plant inspection was completed.

Discussion with staff and acting director: rearranging the infant room and ensuring that the cribs are not located near cords of any type and removing the toys in the cribs for infants above 5 months old.

Violations:
Standard #: 22VAC40-185-160-C
Description: Based on review of six staff's records, the center failed to ensure that all staff had a repeated TB every two years.

Evidence:
1. Staff # 3 and 5 did not have their repeat TB screening. Staff # 3 most recent TB was 6/15/2017 and staff # 5 most recent TB was 9/18/2017.

Plan of Correction: The acting director will send staff to obtain a new TB screening ASAP and will monitor all dates.

Standard #: 22VAC40-185-560-F
Description: Based on observation, the center failed to ensure that when food is provided that a current menu was posted.

Evidence:
1. Observed a menu posted from 9/12-9/16 on the parent board.

Plan of Correction: The acting director changed the menu and will ensure that the cook is changing the menu weekly.

Standard #: 22VAC40-185-560-F-4
Description: Based on observation of lunch, the center failed to ensure that children three years of age or younger may not be offered foods that are considered to be potential choking hazards.

Evidence:

1. Observed 30 children under 3 years that were offered hot-dogs cut into halves and then the pieces were placed into a hot-dog bun. The hot dogs as they were cut into half chunks were a choke hazard to the children.

Plan of Correction: The acting director will revamp the menu not to include hot-dogs. All choke hazards will be cut in fourths.
