Technical Assistance:
1. Reviewed recordkeeping requirements. 2. Discussed/trained vendor on new subsidy requirements. 3. Forms provided: emergency preparedness plan, and proof of identity. 4. Recommend the batteries be changed in the chirping smoke detector.

Comments:
11:10am-12:30pm Conducted an unannounced annual subsidy inspection. There were no children present, and the vendor reported that no children have been enrolled since November 2018. 3 additional adult household members were present in the home today. Inspected the childcare areas of the home and found that toys and equipment have been put away, since there has been no enrollment for a period of months, and that personal household items were throughout. The vendor said the home will be cleaned before children return for care and toys/equipment will be brought out. Discussed daycare practices with vendor. Much of the regulations were marked as not determined today, as the inspector was not able to verify compliance. The vendor is currently certified in First Aid/CPR. Administrative recordkeeping non-compliances found.

Description: Based on interview conducted, it was determined that the vendor has not maintained required records for 5 years after the child left care. Evidence: 1. The vendor said she did not keep a written hard copy record of daily attendance that documents the arrival + departure of each child in care as it occurs, though she looked for attendance records today. 2. The vendor said she did not keep a copy of the children's enrollment records (Child #1, #2, and #3) after they left her care in November 2018, though she looked for the records today. 3. The vendor stated that she was not aware that she needed to keep records.

Plan of Correction: Moving forward, the vendor will keep children's enrollment and attendance records for at least 5 years after the child leaves care

Description: Based on interview conducted, it was determined that the vendor did not maintain required information for 3 of 3 children enrolled in her care in 2018. Evidence: The vendor said she did not keep records for Child #1, #2, and #3. Required information includes: children's full name, sex, address, and date of birth; proof of identity; name home/work address and phone number of parents; name, address and phone number of an emergency contact individual; names of authorized individuals to pick up child; health information; immunizations, written authorization for emergency medical care; written permission to transport the children by car; and other related agreements.

Plan of Correction: Moving forward, the vendor will keep all enrollment documents.
