Comments:
Thank you for your time and assistance with the unannounced renewal inspection conducted by two inspectors on October 22, 2019. The on-site inspection was from 9:45 a.m. to 1:45 p.m. On this date, there were 35 children present in four classrooms and in the care of nine staff members. One extra staff member was available to assist if needed. The total enrollment for the program was 46 children. The exceptions to compliance were noted on the violation notice and the risks ratings were noted on the supplemental page. There were seven staff members' records and five children's records reviewed. Four classrooms were toured and observations were made in three of those rooms. Seven staff members and six children were interviewed. The medication and medication paperwork were reviewed for one child, and two over-the-counter skin products and their authorizations were reviewed. All food was brought from home. The theme for the week was Bats, Owls and Spiders. All interactions between children and staff were age appropriate and positive. Some items that were reviewed and observed included: required posted information, injury reports, documentation of evacuation, shelter-in-place, and lockdown drills, emergency supplies, first aid kit, warm water temperature, updated emergency preparedness plan and injury prevention plan, resilient surfacing, outdoor play, circle time, singing, infant care, centers in the Pre-K room, art activities, floor puzzles, sensory table filled with field corn kernels, infant feeding, diaper changing procedures, hand washing and bathroom procedures, transitions, lunch, and nap time.

Violations:
Standard #: 22VAC40-185-500-B
Description: Based on observation and discussion with staff on 10/22/2019, two staff members failed to clean the diaper changing pad after changing children's diapers.

Evidence:
1. After changing the diapers of children, Staff Member A and Staff Member B did not clean the non-absorbent diaper changing pad with a solution of soap and water before sanitizing the pad.
2. The staff members confirmed they did not clean the changing pad before sanitizing it.

Plan of Correction: During the inspection, the director retrained the staff members on the proper procedures for cleaning and sanitizing a diaper changing pad after changing every diaper. She will follow-up and spot check all staff to ensure they are cleaning and sanitizing the changing pads correctly.

Description: Based on review of five board officers' records and verification from the director on 10/22/2019, the center failed to obtain National Criminal History Checks on all five board officers.

Evidence:
1. The records for Board Officers A, B, C, D, and E did not contain the results of National Criminal History Checks.
2. The Director confirmed that the National Criminal History Checks for the five board officers had not been requested.

Plan of Correction: During the inspection on 10/22/2019, the director sent emails to all five board officers requesting that they immediately register for an appointment to have their fingerprints taken to request National Criminal History Checks. The director will send confirmation to the licensing inspector when all five have submitted the requests and will send the licensing inspector the results as soon as they are received.
