Technical Assistance:
The provider must send documentation to the LI that the background checks have been requested no later than 10 days following this notification dated October 28, 2019. Once the background check is received by the center, the results must be forwarded to the licensing inspector.

Comments:
An unannounced renewal inspection was conducted today from 11:00am-1:10pm. There were 23 children directly supervised by 6 staff and co-op parents. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed eating snack, reviewing the calendar, practicing writing numbers, and playing outside. Staff were observed offering constructive feedback and facilitating smooth transitions between activities and encouraging hand washing. There was an variety of age appropriate materials for the children in care There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, one area outside the center was not maintained in a safe condition.
Evidence:
Protruding metal from the chain link fence was observed on the top of the fence to the left of the gate leading to the playground.

Plan of Correction: We will have this section of the fence repaired.

Standard #: 22VAC40-185-510-A
Description: Based on a review of medication, prescription and non-prescription medication authorizations did not contain all required information.
Evidence:
Medication authorizations for Children 1-4 did not contain the dates for which the authorizations are effective.

Plan of Correction: We will follow up with our Health and Safety Coordinator to obtain the required information.

Standard #: 22VAC40-185-510-G
Description: Based on a review of medication, the center did not follow all procedures to administer prescription medication to children in care.
Evidence:
1. The prescription medication for Child 2 was not in the original container with the prescription label and instructions attached.
2. The prescription medication for Child 4 did not have a parental or physician's authorization. The medication authorization present with the medication was for a different medication that was not present in the center on the date of inspection.

Plan of Correction: We will follow up with our Health and Safety Coordinator to obtain the correct authorizations and medication information.

Standard #: 63.2(17)-1720.1-B-3
Description: Based upon documentation reviewed, an out of state child abuse and neglect result was not obtained.
Evidence:
One of the five staff records reviewed states that Staff 1 has lived out of the state of Virginia in the preceding 5 years, and no out of state child abuse and neglect results were obtained (Staff 5 ; DOH 08/26/2019).

Plan of Correction: We will request a copy of the out of state central registry check.
