Technical Assistance:
Standard 22 VAC 40-185-540 (first aid and emergency supplies) and out of state central registry requests were discussed during the inspection.

Comments:
An unannounced monitoring inspection was conducted on 10/23/19 from 11:55am until 2:50pm. At the time of the tour, there were 47 children in care with 9 staff present. A sample of 5 children's records and 5 staff records were reviewed. Children were observed resting quietly during nap time and participating in afternoon activities. Infant feeding, restroom and hand washing procedures and school bus drop off procedures were also observed. Medication, transportation, first aid and emergency supplies, documentation of emergency practice drills, children's injury reports and center postings were reviewed. Two violations in the areas of physical plant and special care provisions and emergencies are cited on the violation notice and were discussed with the center director during the exit interview.

Violations:
Standard #: 22VAC40-185-280-B
Description: Based on observation, the center did not ensure that hazardous substances shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. A bottle of hand sanitizer was observed on the window sill in the pre-k classroom and was accessible to the children in care.
2. Staff 1 confirmed that the hand sanitizer was stored in an unlocked location.

Plan of Correction: The center responded with the following: This was removed. Going forward all chemicals will be in a locked cabinet unless being directly used. All management and safety captain will ensure chemicals are in locked cabinets during their walk through throughout the day.

Standard #: 22VAC40-185-550-M
Description: Based on record review and interview, it was determined the center did not ensure that the written record of all children's serious and minor injuries contained all of the required components.

Evidence:
1. Nineteen of the twenty injury reports reviewed did not contain the date and time when parents were notified or documentation on how the parent was notified.
2. Staff 1 confirmed that the injury reports did not contain all the required information.

Plan of Correction: The center responded with the following: Management was retrained on 10/23. Going forward all areas of the injury report will be filled out. All management will check to ensure incident reports are filled out correctly on a daily basis.
