Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on October 29, 2019 from 2:55 p.m. through 5:30 p.m. There were forty-one students in the care of five staff. Five children and seven staff records were reviewed and updated. The medication and medication paperwork was reviewed for one child. Areas of non-compliance may be found on the violation notice.
Areas observed were arrival, indoor play, hand washing/bathroom procedures, snack, power hour, cooking activity and departure. Interactions between the children and staff were observed as appropriate and the children seemed comfortable with their daily routine and schedule.

Standard #: 22VAC40-185-60-A Description: Based on review of documentation, the center failed to have a physical within 30 days of enrollment for one child.
Evidence:
1. Five children records were reviewed.
2. Child #3 did not have documentation of obtaining a physical within 30 days of enrollment. Start date was documented as August 19, 2019.
3. The director confirmed the physical was not in the file.Plan of Correction: Documentation of a physical will be requested from the parent.
