Technical Assistance:
We discussed the requirements for staff records. The risk assessments for violations were provided.

We reviewed the requirements for annual training, posting emergency procedure/maps and injury records.

Comments:
Thank you for your assistance during today's unannounced renewal inspection conducted from 10:55 AM to 3:15 PM. Today, there were thirty-two children, ages three through five years, with six to seven staff. I viewed program activities, staffing, supervision, interactions with children, equipment, daily routines, meals, posted information, agency inspection reports, insurance, five records for children, emergency supplies and drill records, asbestos management, five staff records, staff qualifications, staff training, indoor and outdoor areas. I viewed the new playground surface border and the replenished mulch surrounding playground equipment. The teachers have implemented a variety of learning activities such as for fire safety, Halloween, farm life, and counting. Today, some classes enjoyed a visit by firefighters with fire trucks. Also, chapel time included family members. The teachers were observed providing positive guidance and frequent interactions with the children in their group.

Violations:
Standard #: 22VAC40-185-160-C
Description: Based on a review of five records for staff, and an interview with administrative staff, the center failed to obtain the results of an updated tuberculosis screening by two years from the date of the last statement for one staff. Evidence: The TB statement completed on September 1, 2017 for staff member 5 was not updated until September 11, 2019.

Plan of Correction: In the future, the director will remind staff to arrange for their updated TB screenings in advance of due dates so that TB statements are updated by two years from the last statement.

Standard #: 22VAC40-185-70-A
Description: Based on a review of five records for staff, and an interview with administrative staff, there was missing documentation of required training for one staff. Evidence: Staff member 4 began work on November 1, 2018. There was not any documentation of orientation training for initial employment.

Plan of Correction: The director will make sure orientation training forms completed at the time of training are maintained in staff files.
