Technical Assistance:
Discussed the following with the Administrators:
- allowable linens for children
- documentation required for food intolerances

Comments:
An unannounced monitoring inspection was completed today. The physical plant, 5 staff files, 5 children's files, staff training, children's activities, emergency procedures, drills and supplies were inspected between 2:00-3:40pm. There were 15 children under the supervision of 4 staff. During the inspection, the children were observed enjoying free play, listening and dancing to Halloween music and resting during naptime. The children bring their lunches and snacks from home each day they receive care. There are no children with any medication needs during program hours at this time. The interactions between the children and staff were seen to be positive and constructive in nature. Should you have any questions, please contact me at mary.wizbicki@dss.virginia.gov.

Standard #: 22VAC40-185-160-C
Description: Based on file review, 1 out of 5 staff records does not have a TB Test/screening completed within the past two years.
Evidence
1. Staff #1's most recent TB test/screening was completed on 9/18/17.

Plan of Correction: This will be completed again.

Standard #: 22VAC40-185-320-B
Description: Based on observations, a restroom used by 2-3 year old children was not equipped with all required items.
Evidence
1. The bathroom had a toilet stall that did not have toilet paper accessible.
2. There were no paper towels accessible.

Plan of Correction: Corrected during inspection.

Standard #: 22VAC40-665-740-B-6
Description: Based on observations, the center did not ensure that diapers were disposed in a storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal.
Evidence
1. The disposal system for diapers in the Infant room requires the staff to use their hands to touch the exterior surface.

Plan of Correction: A foot operated trash can will be placed in the room for diaper disposal.

Standard #: 63.2(17)-1721.1-D
Description: Based on file review, 4 out of 5 staff records do not have a completed search of the central registry (SCR) in any state they have lived in within the past 5 years.
Evidence
1. Staff #2-5 do not have a completed SCR for the state of Virginia.
2. Staff #2 does not have a completed SCR for North Carolina.
3. Staff #5 does not have a completed SCR for Washington DC

Plan of Correction: These background checks were sent, but there was no documentation of a follow-up within 30 days. A follow-ip was completed on Monday, 10/21. However, since the agency stated that they did not have these requests, the administrators will submit these again.
