An unannounced monitoring inspection was completed on October 23, 2019 from approximately 10:30 a.m. to 12:45 p.m. Sixty-seven children were in care and approximately eighteen staff were on premises during the inspection. Two classrooms were on a field trip. Staff and children were observed in classrooms and on the playground engaged in activities to include activities for infants, outside play, lunch time, and transitioning to nap time. Interviews and interactions were conducted and medications were reviewed. Seven children?s records and six staff records were reviewed. The owner and director were available for the inspection and the director was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Standard #: 22VAC40-185-70-A Description: Based on a review of staff records and interview, the licensee failed to maintain all the elements of staff?s records for one of six staff.
Evidence: 1. Staff #6's record, hired on August 5, 2019 as a program leader, lacked documentation of the training required for the job position. The record contained documentation of approximately 10 hours of the required 24 hours to be a qualified program leader. 2. In an interview with Administrator #1, she reports that Staff #6 completed the required amount of training at her previous job within the past 6 months but acknowledged there is no documentation of this training in the record.Plan of Correction: The director will obtain documentation of the training from the center where the staff previous worked.
