Comments:
An unannounced renewal inspection was completed on 10/30/2019 from 9:25 A.M.-3:30 P.M. At the time of the inspection 62 children were in care with eleven staff, one volunteer, and two administrators present. An additional 51 children arrived afterschool and were in the care of four staff members. Six children records, eight staff records, one volunteer record, two board members background checks, four medications and required paperwork, first aid kits, emergency supplies, health inspection, outdoor playground, posted required information, injury prevention plan, and parent handbook were reviewed. Children were observed during infant feeding, diapering,one-on-one infant interactions, outside playtime, behavioral guidance, transition, free play activities that included building, blocks, cars, and fine motor activities, music and movement, story time, handwashing, lunchtime, and naptime and arrival of afterschool children. The exceptions to compliance are noted on the violation notice.

Standard #: 22VAC40-185-270-A
Description: Based on observation of the classrooms, the center failed to ensure that all areas and equipment were maintained in a clean, safe, and operable condition.

Evidence:
1. In Classroom #1 there was paint peeling and was accessible to children.
2. Staff verified that there was paint peeling in Classroom #1 and was accessible to children.

Plan of Correction: Administration will contact a contractor to get Classroom #1 repainted.

Standard #: 22VAC40-185-570-E
Description: Based on observation, the center failed to ensure that all prepared infant formula was labeled with the date.

Evidence:
1. In Classroom #2 there were four bottles with prepared formula that were not labeled with the date.
2. Staff verified that the bottles were not labeled with the date.

Plan of Correction: Staff immediately labeled all bottles with the date, in the future a staff member will be responsible for ensuring that all prepared bottles brought into the center have the date labeled on them.
