Comments:
An unannounced Monitoring inspection was conducted in the Center on October 29, 2019. The LI arrived to the program at approximately 8 AM and departed at 12:00 PM . The LI observed 42 children grouped in 4 classrooms. The ages of the children ranged from 10 months to 5 years. The children were observed during arrival, breakfast, free play circle time, and lunch.
A sample of 5 children's records was reviewed. Medications were observed in the center. Documentation of the annual fire and health inspections was current.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on review of five children's records, the center failed to ensure that all records were complete.

Evidence:
1. Child # 3 most recent enrollment form had the child's father as an emergency contact. The emergency contacts are to be someone that the center can contact when the parent's can not be located.

Plan of Correction: The FSW updated the record during the inspection with the parent approval. The FSW will monitor all records.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the center failed to ensure that all areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

Evidence:
1. Observed in the four year old room, a rug that had a tear on the edge of the rug and posed a tripping hazard.

Plan of Correction: The rug was removed during the inspection. The areas will be monitored by staff.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center failed to ensure that hazardous substance were kept in a locked place.

Evidence:
1. In the four year old room, the locking mechanism on the cabinet was not locked. There were several containers of cleaning chemicals in the cabinet. The cabinet was approximately 6 foot off the ground and was not accessible to the children in care.

Plan of Correction: During the inspections the teacher locked the cabinet and staff will monitor.

Standard #: 22VAC40-185-570-A
Description: Based on observation, the center failed to ensure that when a child is placed in an infant seat or high chair, the protective belt shall be fastened securely.

Evidence:
1. There were four infants sitting in high chairs that were approximately two feet off the ground. All four infants did not have the protective belts fastened securely.

Plan of Correction: The belts will be fastened as soon as the infants are placed in the high chair even if the parent places the child in the chair.
