Comments:
The licensing inspector conducted an unannounced monitoring inspection 10/16/2019 from 9:45am to 1:00pm. The children were observed learning through play in centers, participating in story time, counting, coloring and eating lunch. The lunch menu was posted. Lunch consisted of beef ravioli, broccoli and cheese, pears and milk. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms were inspected today. The facility was equipped with manipulatives and supplies, and items were available to the children. Medication is not administered at this time. However, the center has several MAT trained staff.


Five children?s records and five employee records were reviewed during this inspection. Ten injury reports were reviewed.

The center's first aid kit and emergency supplies were inspected.

The violations from the previous inspections were checked for corrections. There were no repeat violations found during today?s inspection.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on record review, the center did not ensure one of five children's records had documentation of immunizations before the child can attend the center.
Evidence:
The record of child #3 (start date: 7/8/2019) had documentation of immunizations dated 8/1/2019.

Plan of Correction: The Director will collect future immunization forms before the students begin at the center.

Standard #: 22VAC40-185-340-F
Description: Based on observation, the center did not ensure that children under 10 years of age always shall be within actual sight and sound supervision of staff.
Evidence:
During the inspection staff #1 (start date: 3/7/2018) walked out of the pre-k / preschool age classroom to answer the center's front door, leaving the children alone, on three separate occasions.

Plan of Correction: Staff will make sure one staff member is in the classroom before leaving the room, subject to ratios.
