Comments:
The licensing inspector initiated an unannounced complaint inspection in the center on September 18, 2019 in response to a complaint received by the licensing office regarding physical plant and staffing and supervision. Review of center documentation, interviews with staff members and observations were conducted. Based on information gathered during the investigation, there was not a preponderance of evidence that supports the allegation of staffing and supervision, however there was a preponderance of evidence to support the allegation regarding physical plant therefore the complaint is determined to be Valid. Areas of non-compliance are noted ion the Violation Notice. The results of the inspection were reviewed the the Center Director during the exit interview.

Description: Based on observation, the licensee did not ensure areas and equipment of the center, inside, were maintained in a clean, safe and operable condition.


Evidence:


1) In the toddler classroom restroom, two of two toilets were not operable. The toilets were covered by garbage bags. There was a plastic square on one toilet and a purse on the other toilet. Staff stated the area is used for storage.

2) In the two year old classroom restroom, one of the two toilets was not operable. The toilet had a broken handle with sharp edges, the seat cover was detaching and the toilet was clogged with streamers and other items.The restroom was not maintained clean and safe it had caution sign on floor possible tripping hazard, papers on the floor , diapers on the floor, bag of wipes with a plunger on top of it behind the toilet,diapers and toys mixed up in a a basket and toilet paper on the floor.

3) In the Pre-K classroom one of the two toilets were not operable. In the girls restroom, the toilet handle was broken and was not able to be flushed. Staff stated it has been broken for three weeks.

4) In the Preschool classroom two of the four restrooms were not operable. In the girls restroom, there was a toilet covered with a trash bag and in the boys restroom, there was one toilet covered with a trash bag. Staff stated the toilets were not operable since 8/26/2019 and work orders have been submitted.

Staff #7 stated there was two work orders submitted for two toilets. Staff #7 stated It takes up to 30 days to get fixed . During the inspection a plumber arrived to fix the two toilets that the work orders were submitted for and staff discussed with plumber the additional toilets that were not operable.

Plan of Correction: 270-A: All toilets were repaired the day of the visit while the plumber was on site. Moving forward, the management team will ensure that work orders are escalated to emergency status when it involves broken parts or multiple toilets being down at the same time. The safety captain will be checking daily for operational issues and broken parts. In addition, as of 10/11/19, all staff have been retrained by the new Center Director on proper bathroom cleanliness and sanitation procedures, as well as how to properly store items in the bathroom areas. New CD has a system in place to ensure that all new staff are also being trained on this as they are hired and our new school housekeeper is a part of this training plan.
