Comments:
The licensing inspector conducted an unannounced monitoring inspection on Tuesday, October 29, 2019 between 11:00 am to 2:00 pm determine the center's compliance with licensing standards. During this inspection, the children in care were observed by the licensing inspector in the various classroom settings. The census for today's inspection consisted of 104 children with 20 staff present including volunteers and administrative staff. The licensing inspector completed observations of the children in care completing a variety of activities to include participating in outdoor play, working in various centers, interacting with peers and staff, participating in circle time, lining up and transitioning outdoors for play activity, completing bathroom breaks/handwashing, eating lunch, brushing their teeth, and preparing for napping. The children's lunch today consisted of cheese ravioli, lima beans, diced pears, whole grain garlic breadsticks, and milk. The center's classrooms, diapering station, bathrooms,designated childcare areas were inspected by the licensing inspector. The required posting were reviewed by the inspector. There were eleven children?s records reviewed during this inspection. The center's first aid kit and emergency supplies were inspected. Medication is being administered, medications, and written authorizations were reviewed for the center's compliance. Standard #: 22VAC40-185-140-A
Description: Based on review of children's record and interview with staff, the center did not ensure to obtain physical examination within one month after attendance.

Evidence: In Child#11's record (start date: 9/03/19) a physical examination was not found. Interview with staff confirmed the physical was not obtain for the child and a request was submitted on 10/21/19.

Plan of Correction: The center will contact case manager to follow up on status of physical with the parent.
