Comments:
The licensing inspector conducted an unannounced monitoring inspection, at which play groups, activities, music, art, personal hygiene, and lunch time were observed. Children were observed; and staff and children files and other documentation were reviewed. Ninety-eight (98) children, aged 4 months-5 years, were found to be in care. Previous violations were reviewed, and one (1) of the violations from the previous inspection had not been corrected. Ratios continue to be an issue, and need to be maintained for future compliance. Please contact me if you need further assistance. Thank you for your cooperation. This inspection began at 8:40 am and concluded at 12 noon.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on review of the outside playgrounds the facility failed to maintain compliance with this standard as required regarding a tripping hazard.

On the big playground the rubber matting used as resilient surfacing is starting to buckle all over, causing the corners to protrude upward all over the space. This causes a tripping hazard to the children and staff while using the playground.

Plan of Correction: Administration and maintenance will be contacted and informed of the hazard and a decision will be made as to how best to correct the issue for safety and compliance.

Standard #: 22VAC40-185-330-B
Description: Based on review of the playgrounds the facility failed to maintain compliance with this standard as required relating to a fall zone with resilient surfacing.

Evidence:
On the small playground there is a plastic climber with a small slide that is over bare ground and not over resilient surfacing as required.

Plan of Correction: The piece of equipment will be removed until it can be used within the guidelines of compliance.

Standard #: 22VAC40-185-340-F
Description: Based on observations the facility failed to maintain compliance with this standard as required relating to children being in sight and sound of a staff member.

Evidence:
During the inspection there was no staff present with the soccer shots individual when he had a small group of children doing soccer lessons in the open area outside of the classrooms. The time frame was approximately 9:30 am-9:40 am. At 9:40 am the cook stayed with the group for compliance.

Plan of Correction: An employee is usually with the group during this time, however due to a staffing issue there wasn't an available employee for those 10 minutes. We are working on hiring open positions and re adjusting the schedule to ensure future compliance.

Standard #: 22VAC40-185-350-E-4
Description: Based on observations and discussions with staff the facility failed to maintain compliance with this standard as required relating to ratios. This is marked as a repeat violation as non-compliance with this standard was previously cited on 3/21/2019.

Evidence:
From approximately 9:15 am-12 noon the 3 preschool classes were out of ratio. Each class had 11 children with 1 staff. The ages of the children were 3 years-5 years.
Non-compliance of the ratio with this age group was previously cited on 3/21/2019, and is considered a repeat violation.

Plan of Correction: An employee is usually with the group during this time, however due to a staffing issue there wasn't an available employee during this time. We are working on hiring open positions and re-adjusting the schedule to ensure future compliance.

Standard #: 22VAC40-185-540-C
Description: Based on review of the first aid kit the facility failed to maintain compliance with this standard as required relating to items required to be in the kit.

Evidence:
There were no tweezers available in the first aid kit as required.

Plan of Correction: Tweezers will be added to the first aid kit for compliance.
