Comments:
An unannounced subsidy health and safety inspection was conducted on 10/23/2019 from 2:30 pm - 4:00 pm. A renewal inspection was concurrently conducted at this facility is a licensed child day center.
There were 25 children, ages three years - school age, in care with seven staff. The preschool class was observed during outdoor play and making an art project. The school age class was observed during arrival from public school.
Records were reviewed for seven children and four staff.
Medication and emergency supplies also reviewed.

Standard #: 22VAC40-665-540-A
Description: Based on record review, the vendor failed to ensure that the TB screening for staff shall have been completed within the last 30 calendar days of the date of employment.

Evidence:
1. The TB screening for staff 1 was completed on 09/30/2019. Staff 1 has a hire date of 09/09/2019 and the TB screening was conducted on 09/30/2019.

Plan of Correction: Going forward all new staff will obtain a TB screening prior to employment and within the 30 day time frame.
