Technical Assistance:
Reminded management staff of the importance of knowing the location and meaning of all required administrative documents, including documents required by the Subsidy program.

Comments:
An unannounced full renewal inspection was conducted today. There were 50 children in care, with a total of 5 staff members assigned to their direct supervision. Children's records, the physical plant, emergency preparedness items, classroom programming, staff:child interactions, children's medications, and staff training were inspected today.

The children were observed playing flag football, engaging in creative play activities, making bead jewelry, and playing with toys. All children brought lunch from home today.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on a review of children's health records, it was determined that the center did not obtain documentation of a current immunization record before the child began attending the center.
Evidence:
Child record B does not include documentation of an immunization record. This child entered care on 8/28/17.

Plan of Correction: The health forms must have been misplaced when the parent completed new registration paperwork. I will ask the school nurse for a copy of the child's shot record.

Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records, it was determined that three records do not contain all required information.
Evidence:
1 - Child record B does not include "authorized pick ups" information.
2 - Child record C does not include: the father's work contact information; a second emergency contact that is not the parent.
3 - Child record C does not include "authorized picks ups" information.

Plan of Correction: Each parent will be asked for the information.

Standard #: 22VAC40-185-550-M
Description: Based on a review of written records of children's injuries, it was determined that each record does not include all required information.
Evidence:
Of the injury reports completed for this school year, three of them do not include at least 2 staff/parent signatures.

Plan of Correction: All staff will be reminded to complete the form correctly before it is filed away.
