Comments:
A mandated monitoring inspection was conducted today, 11/4/19. There were 16 children present with 2 staff. Children were observed reading, building with colored magnetic shapes, coloring, making construction paper houses, making rainbow loops, and learning to crochet. Reviewed 3 children's files, 2 staff files, attendance, emergency evacuation log and plan, medications and authorizations and physical plant. Violations:
Standard #: 22VAC40-185-140-A
Description: Based on record review, each child did not have a physical exam within 30 days of their first day of care.
Evidence:
Child#2 (start date 8/17/19) did not have documentation of a physical within 30 days.

Plan of Correction: I will get it from the school

Standard #: 22VAC40-185-60-A
Description: Based on record review, each child's record did not contain documentation of child updates and confirmation of up-to-date information
Evidence:
Child#1 and Child#2 were missing documentation that the file had been updated for the new year. Last update for both records was 8/28/18.

Plan of Correction: I will have them update the information the next time they come in.
