Comments:
An unannounced inspection was conducted from 8:50 ? 10:20 am to investigate a complaint regarding Staffing and Supervision and Special Care Provisions and Emergencies. Based on evidence obtained through classroom observations and interviews with staff, the complaint was considered valid. Standard #: 22VAC40-185-350-E-4
Complaint related: Yes
Description: Based on observation, the center failed to maintain the required ratio of one staff member for every ten children, ages three years to the age of eligibility to attend public school. Evidence: At approximately 10:00 am, there were 22 preschool children (3-4 years of age) being supervised by two staff members.

Complaint related: No
Description: Based on observation, and staff interview, it was determined that the center was casually reassigning children from their regular group for reasons other than administrative necessity disrupting the children's schedule and attachment to their regular group. Evidence: The LI observed a crying child (age 3) being reassigned to a two year old classroom when the preschool classroom exceeded it's ratio. Staff interviews revealed that children are reassigned regularly, whenever necessary to maintain ratios. Child #2 has been reassigned to a different age group but does not have a written assessment completed by the director and program leader.

Complaint related: Yes
Description: Based on observation, the diapering surface is not being cleaned and sanitized, as required, after each use. Evidence: After changing a diaper, staff sprayed the surface with a bleach water solution but did not first clean the surface with soap and water.
