Comments:
An unannounced monitoring inspection was conducted November 4 2019 between 11:00am and 12:15pm. There were 58 children in the care of 5 staff members at the start of the inspection. Children were observed washing hands, eating lunch, completing art projects, working on academic websites, and reading. The site is decorated with community related themes including Theater, Engineering, and Exercise. Areas of non-compliance are listed on the violation notice. Standard #: 22VAC40-185-420-E-3
Description: Based on record review, 2 of 6 children's records reviewed did not contain documentation that the center requested, at least annually, that the child's record is up to date.

Evidence: Child #1 record last had a documented date of 8/29/2017. Child # 3 record last had a documented date of 08/29/2017.

Plan of Correction: Will be completed in the next two days.
