Since staff records are currently being housed off site, please continue to keep staff emergency contacts on site. It would also be very helpful if a copy of Staff?s CPR and First Aid Certification, Daily Health Observation Training and (as applicable) Medication Administration Training (MAT) be kept on site for review since it is required that a staff with these current trainings be present with the children at all times, whenever they are in care.

An unannounced monitoring inspection was conducted on 10/23/19 from 3:15 ? to 4:25 p.m. to monitor the program?s compliance with the Standards during their first 60 days of operation under the 6 month Conditional Licensure period. The inspection was continued on 10/24/19 from 12 noon, until 2:30 p.m. with an unannounced review of staff records at the AlphaBEST administrative office located off site.
At the time of entrance on 10/23/19 the following adult to child ratios were observed: 7 children ages 5 to 12 years old, with 2 staff
The records sample size consisted 2 staff records and 5 children?s records. Children were observed playing table top games, creating a holiday craft project, and eating an afternoon snack consisting of Ham and Cheese sandwiches with lettuce, Pickle spear, apple slices, Cheetos and milk. Restroom procedures, hand washing and table sanitizing were also observed. Medication administration was not observed. No medication is currently being administered to children in care. Staff reported no accidents or injuries having occurred since the last inspection. Violations cited appear on the violation notice. Thank you for your time during today's inspection. Standard #: 22VAC40-185-60-A
Description: Based on a review of records and interview, the center did not maintain and keep at the center a separate record for each child enrolled which contained all of the required information.
Five children's records were reviewed during the inspection.
The record for child #2 did not contain addresses for the two designated people to call in an emergency if a parent cannot be reached.
Staff #2 stated that this information was not in the record.

Plan of Correction: Staff #1 stated that she would have the parent to complete this information on the enrollment form at the time of pick up or contact them via phone to obtain the addresses.

Standard #: 22VAC40-185-70-A
Description: Based on a review of records and interview, the licensee was unable to demonstrate that a complete record containing all required documentation is maintained for each staff.

Evidence: Two (all) staff records for the program at this site were reviewed.
The record for staff #2 did not contain documentation that two or more references as to character and reputation as well as competency were checked before employment or volunteering.
The area manager was unable to provide evidence that reference checks had been completed for staff #2.

Plan of Correction: The area manager stated that this documentation may be located at the corporate office. If the documentation is located, a copy will be placed in the staff's record. Otherwise, references will be checked and documented in the record.

Standard #: 22VAC40-191-60-C-2
Description: Based on a review of records and interview, the licensee was unable to demonstrate that the results of a Search of the Central Registry has been obtained for each staff within 30 days of employment.
Two (all) staff records for the site were reviewed.
The record for staff #2, whose date of employment was documented as 9/13/19, did not contain results of a Search of the Central Registry
The area manager stated that Central Registry search requests were completed at the time of hire for these staff, but that when results had not been returned, new forms were completed for each staff and re-mailed on 10/2/19.

Plan of Correction: They were requested again and mailed off on 10/2/19.
Results will be filed in staff records as they are received back.
