Comments:
The licensing inspector conducted an unannounced monitoring inspection on Monday, November 4, 2019 from 3:40pm to approximately 6:05pm. There were a total of 94 children present at the center with nine staff supervising the children. Additional staff were available to the children throughout the inspection. During the inspection, children were observed completing homework, playing in the cafeteria, taking bathroom breaks, eating an afternoon meal. The children were later observed playing outside and interacting with staff and peers. The afternoon meal consisted of BBQ popcorn chicken, a bun, broccoli, carrots, ranch dressing, apples, and milk. All areas of the center including the cafeteria, hallways, bathrooms, and playgrounds were inspected. The center was equipped with toys and supplies and items were available to the children. The following information was reviewed: emergency supplies, injury reports, fire and shelter-in-place drills, daily attendance, menu, required postings and parent agreements. One bus and two minivans were inspected today. Medication is administered and medication and medication authorization forms were reviewed. During the inspection, ten children?s records were reviewed. Staff information required to be present at the center for staff members were reviewed. Staff records are kept off-site and will be reviewed by licensing inspectors at a later time. The most recent evacuation drill was documented on 10/15/2019. The most recent shelter-in-place drill was documented on 09/04/2019.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of ten children's records on 11/04/2019, the center did not ensure two children's records contained the required information.

Evidence: 1) The record for Child #2, enrolled on 09/16/2019, did not contain a complete address for the two emergency contacts listed. 2) The record for Child #10, enrolled on 10/16/2019, did not contain an address for the second emergency contact listed. Each child record shall contain the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.

Plan of Correction: Per the Center: "One child was still present, mom came and corrected file. Second child's record will be corrected as soon as possible."

Standard #: 22VAC40-185-510-D
Description: Based on a review of six medications and interviews on 11/04/2019, the center did not ensure that two medication authorizations were available to staff during the entire time it was effective for two out of four children.

Evidence: 1) A non-prescription allergy medicine was observed for Child #13 and no medication authorization was present for this medication. 2) A prescription inhaler was observed for Child #14 and no written parental authorization was present for this medication. 3) During interview, a member of management reported the authorizations for these medications were not present.

Plan of Correction: Per the Center: "Will audit of all medications and will make correction as they come up."
