Comments:
An unannounced monitoring inspection was conducted between the hours of 8:00am and 10:00am. Upon arrival there were 10 children in care with 2 staff present. Children were observed in centers and singing. A sample of 3 staff and 2 children's records were reviewed.

Violations:
Standard #: 22VAC40-185-160-C
Description: Based on a record review the center did not ensure all staff had repeat tuberculosis screenings, every 2 years since the last one.
Evidence:
Staff #1's last tuberculosis screening was dated 08/04/17.
Staff #3's last tuberculosis screening was dated 08/11/17.

Plan of Correction: The following corrective action plan was provided by the administrator during the exit interview:
This was an oversight. Both staff will obtain new TB screenings this week.

Standard #: 22VAC40-185-60-A
Description: Based on a record review the center did not ensure all required information was kept for each enrolled child.
Evidence:
Child #2 did not have documentation of having the addresses of a minimum of 2 emergency contacts or a proof of identification.

Plan of Correction: The following corrective action was provided by the administrator during the exit interview:
A copy of the birth certificate and emergency contacts will be obtained from the parent this week.
