63.2 Child Abuse & Neglect
An unannounced monitoring inspection was conducted today from 9:00am to 1:30pm with the director. There were 69 children in care, ranging from three months to 10 years of age, supervised by 10 staff. The children were observed reviewing colors and shapes, coloring turkeys, reading a book about police, dancing to a song about dinosaurs, playing on the playground, singing songs, and playing musical chairs. Five children?s records and five staff records were reviewed. The Center has staff certified in CPR, First Aid, and Daily Health Observation. Two medications and authorization forms were reviewed and the center has four staff current in Medication Administration Training (MAT). The attendance and emergency drill log were reviewed.The most recent evacuation drill was documented as 10/29/19 and the most recent shelter-in-place was documented as 3/26/19. Breakfast today was yogurt with granola, blueberries, and milk. Standard #: 22VAC40-185-160-A Description: Based on review of five staff records, the center did not obtain documentation of a tuberculosis (TB) test or screening for staff within 21 days of hire or within 12 months prior to hire. Evidence: There was no documentation of a TB test or screening for Staff B, with a start date of 9/3/19; Staff C, with a start date of 9/18/19; Staff D, with a start date of 7/1/19; or Staff E, with a start date of 12/27/18. Plan of Correction: The staff will obtain their TB test or screening and we will have documentation in their files by 11/12/19. Moving forward, staff will not be able to start until they have obtained their TB test.
Standard #: 22VAC40-185-70-A Description: Based on review of five staff records, the center did not ensure that each staff record contained all of the required documentation. Evidence: The records for Staff A, Staff D, and Staff E did not contain documentation that two or more references as to character and reputation as well as competency were checked before employment. Plan of Correction: I will call the references again and put the documentation in the file.
Standard #: 22VAC40-185-280-B Description: Based on observation, hazardous substances were not kept in a locked place using a safe locking method that prevents access by children. Evidence: In the three year old classroom there was a spray bottle of bleach and water on top of the paper towel dispenser, accessible to children. Plan of Correction: The spray bottle was moved during the inspection.
Standard #: 22VAC40-191-60-B Description: Based on review of five staff records, the center did not have documentation of a completed sworn statement or affirmation for each staff prior to date of hire. Evidence: The record for Staff B, with a start date of 9/3/19, did not contain documentation of a sworn statement. Plan of Correction: Staff B completed the form during the inspection. Moving forward, I will make sure items are put in files immediately before the staff starts.
Standard #: 22VAC40-191-60-C-1 Description: Based on review of five staff records, the center did not have documentation of the results of a central registry search within 30 days of employment of a staff member. Evidence: The records for Staff D, with a start date of 7/1/19, and Staff E, with a start date of 12/27/18, did not contain the results of a central registry search. Plan of Correction: We sent the central registry checks in the middle of September and resent them on 10/17/19 due to a check error. We will follow up if they are not received by 11/15/19. Moving forward, we will follow up and document if the results are not received within the time frame.
