Technical Assistance:

Discussed more resilient surfacing options for the playground with the Director.

Comments:

An unannounced monitoring inspection was conducted today from 8:55am to 11:30am with the Director. There were 19 children in care, ranging from two to four years of age, supervised by two staff. The children were observed playing on the playground, transitioning inside, and playing in centers. Five children?s records and three staff records were reviewed. The Center has staff certified in CPR and First Aid. There is currently no medication on site, but the center has staff current in Medication Administration Training (MAT). The attendance and emergency drill log were reviewed. The last evacuation drill was conducted 10/18/19 and the last shelter-in-place was conducted on 9/9/19. The first aid kit was observed. Standard #:	22VAC40-185-140-A
Description:	Based on review of five child records, each child did not have a physical examination by or under the direction of a physician within one month of attendance. Evidence: Child A, with a start date of 8/5/19, did not have documentation of a physical in their record.
Plan of Correction:	I will speak to mom today and make sure it is in the file by Friday.
Standard #:	22VAC40-185-160-C
Description:	Based on review of three staff records, the center did not ensure that staff members obtained and submitted the results of a follow-up tuberculosis (TB) test or screening at least every two years from the date of the first initial screening or testing. Evidence: The most recent documented TB test or screening for Staff A was dated 2/7/17 and for Staff B, the most recent documented TB test was dated 7/1/16.
Plan of Correction:	I will make sure we get them updated by the end of next week.
Standard #:	22VAC40-185-240-D-5
Description:	Based on review of three staff records and interview with the director, the center did not ensure that there is always at least one staff member on duty who has obtained within the last three years instruction in performing the daily health observation of children. Evidence: There was no documentation of any staff obtaining daily health observation training within the past three years.
Plan of Correction:	The Director has had the training in the past, but will schedule to go again within the next month.
Standard #:	22VAC40-185-280-B
Description:	Based on observation, hazardous substances, such as cleaning materials, were not kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. In the two-year-old classroom there was a spray bottle of bleach and water on a shelf above the teacher area and a spray bottle of disinfectant in an unlocked upper cabinet. 2. In the three-year-old classroom there was a spray bottle of disinfectant on an upper shelf in the bathroom. 3. In the four-year-old classroom there was a spray bottle of disinfectant spray on top of a cabinet. 4. In the school age classroom there was a bottle of green algae eliminator pool and spa cleaner on top of a cabinet.
Plan of Correction:	I locked them up already.
Standard #:	22VAC40-185-350-E-3
Description:	Based on observation and staff interview, the ratio requirement for two-year-olds of one staff for every eight children was not met. Evidence: At 9:00am on 10/28/19 there were three two-year-olds and six three-year-olds with one staff.
Plan of Correction:	Another staff arrived at 11:00am so the room will now be in ratio.
