Comments:
A Subsidy Health and Safety monitoring inspection was conducted on 11/6/2019 to review supplemental health and safety requirements. This inspection began at 10:20am and ended at 1:15pm. There were 11 staff with 60 children in 6 classrooms at the time of the visit. At the start of the inspection, children were observed playing outside, coloring, painting and other organized activities. Children's files, emergency preparedness procedures, fire drill log were reviewed. The physical plant and playground were inspected. There were five medications on site at this time. A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Thank you for your cooperation today. Standard #: 22VAC40-665-540-A
Description: Based on record review, a tuberculosis (TB) screening provided by staff was not completed within 30 calendar days prior of the date of employment.
Evidence: Staff #3 (date of hire: 8/12/2019) TB screening is dated 3/25/2019.

Plan of Correction: We will have her obtain an updated TB screening within the next two weeks.
