Technical Assistance:
Technical assistance was provided regarding sending in the renewal application, medication authorization forms and following up on background checks if they are not received back within 30 days. The central registry check is for staff #1 needs to be followed up on, forward results to inspector within 10 business days.
Child's potty chair under teacher's desk should be stored in the bathroom.

Comments:
An unannounced renewal inspection was conducted today from 10:00am-12:30pm. There were 23 children supervised by 3 direct staff. The physical plant, staff records, children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading and participating in a math lesson. Staff-to-child ratios were in compliance. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Description: Based on observation, not all medication or other harmful agents are stored in an area not accessible to children.
Evidence:
An over the counter medication was observed in an open purse on the floor in the 4 year old's classroom.

Plan of Correction: We will move the purse to a locked closet.

Standard #: 22VAC40-185-290-3
Description: Based on observation, not all electrical outlets have protective covers.
Evidence:
An outlet located in the 3 year old's classroom was uncovered, accessible to children.

Plan of Correction: We will make sure all outlets are covered.

Standard #: 22VAC40-185-380-A
Description: Based on observation, a daily schedule of activities is not posted.
Evidence:
There is no daily schedule posted in the 3 and 4's classroom.

Plan of Correction: We will post them when we complete the updates.

Standard #: 22VAC40-191-60-C-2
Description: Based on review of five staff records, one does not have documentation of a central registry finding within 30 days of employment.
Evidence:
Staff #1, date of hire 9/13/19, does not have documentation of a central registry finding within 30 days of employment.

Plan of Correction: We sent it in, it was returned, we sent it back and have not received it back again.

Standard #: 22VAC40-665-520-B-9
Description: Based on review of three medications, three do not have medication authorizations.
Evidence:
Child #1 has three medications on site without medication authorization forms.

Plan of Correction: We will get the medication authorization forms for the medications.
